Wellopp Launches Community Services Referral Team to Enhance its Social Determinants of Health Solution

Wellopp, a patient engagement technology company focused on impacting social determinants of health to improve patient outcomes, recently announced the launch of its new community referral services team.

Wellopp Connect works closely with providers, payers, hospitals, and pharmacies to develop a customized, local network of community resources focused on improving patients’ social determinants of health (SDOH) factors. This network extends much-needed support to help minimize barriers to care, reduce preventable readmissions, improve health outcomes, and lower downstream healthcare costs.

“We know that as much as 60 percent of a patient’s health is driven by factors occurring after the patient leaves the hospital or clinic; we help identify the barriers to successful outcomes and connect patients to needed resources,” says Thomas L. Lauzon, President and CEO of Wellopp. “Wellopp Connect leverages our technology platform to provide a personalized approach to improving the health and well-being of the patients we serve.”

The Wellopp interactive experience begins with a digital screening, developed by clinicians and data scientists, that’s delivered to patients at any point of care, such as in a clinic or at discharge. The questions, aimed at identifying SDOH factors, are delivered in clear, simple language that patients can understand. This information then becomes part of the patient record, seamlessly integrating into the health system’s workflow, enabling care teams to identify at-risk patients and intervene early enough to impact health outcomes.

Now, with the addition of Wellopp Connect, care teams are empowered to continue addressing SDOH factors after patients leave the hospital or provider’s office. Wellopp carefully curates a local network of food banks, transportation services, career support services and other valuable organizations, and follows through to see that those connections happen and to ensure patients receive the services they need.
