While radiology remains the major contributor to any enterprise imaging IT strategy, most approaches now have a broader scope than Digital Imaging and Communications in Medicine (DICOM) images from radiology alone.•The new technology paradigm that has been emerging in recent years is based on centralized archiving on the back end, combined with distributed viewing using enterprise viewers on the front end.•Providers are primarily taking one of two approaches to implement such an enterprise-oriented strategy. One approach relies on PACS-neutral archives from third-party vendors, commonly referred to as vendor-neutral archives (VNA), and the other one relies on enterprise PACS archives, mainly provided by incumbent PACS vendors.•Although they have been around for about ten years now, VNAs have only been expanding slowly into the marketplace, as they have been following a gradual top-down expansion starting from large-scale organizations and select early adopters.•As for the development of an enterprise PACS archive involving (one of) their incumbent PACS vendor(s), they have been limited in time due to the late entry of vendors into this space, and limited in scope due to the fact that they involve primarily highly satisfied PACS customers and one of a select set of commercially available solutions.•As the two approaches make their way into the marketplace, they are creating a sizeable and fast-growing segment of the revenue opportunity for imaging informatics and PACS.
