One Source Pharmacy evaluated a number of software solutions to support its initiative and selected MirixaEdge because of the flexibility of its underlying web-based platform, MirixaPro(SM), and the availability of comprehensive customized templates for patients with chronic conditions such as diabetes, asthma, and cardiovascular disease. With MirixaEdge, One Source Pharmacy can collaboratively work with its client, FPG, and FPG patients to monitor chronic disease states, track progress with drug regimens and educate patients.

Devan Patel, RPh PharmD, One Source Pharmacy Director notes, "Our goal is treat all of our patients like family and ensure they stay happy and healthy. MirixaEdge provides us with an excellent tool to manage our patients' medication needs more effectively so they can enjoy a higher quality of life."

Edwin Valentin, RPh, MTM-C Pharmacist at One Source Pharmacy, explains: "Adding data collection and patient education services around medications to our medical home will create an even more complete picture of the patient's health status. Physicians and care givers will be more informed and positioned to make optimal care decisions." Rosalie Baez, PharmD, BCPS, FPG Pharmacy Services Director states, "We are excited to partner with One Source Pharmacy and to use Mirixa technology to support our collaborative care delivery model."

Rick Solano, CEO of Mirixa comments: "As health care delivery models such as the medical home and accountable care organizations are formed, greater collaboration between care givers, including pharmacists, is essential."  Solano continues, "Managing appropriate use of medications is an important facet of total care management. Mirixa is pleased that health plans, employers, PBMs, pharmacies and now, forward-thinking organizations like FPG and One Source Pharmacy have embraced our solutions."

Nationally recognized for excellence, FPG's Disease Management Department recently received the National Committee of Quality Assurance's (NCQA) Full Accredidation, a distinction widely considered the gold standard for evaluating health care quality and value. NCQA also recognized nearly 40 of FPG's physicians as among the best in diabetes care in the nation. The incorporation of MirixaEdge into their Medical Home model is an example of FPG's continuous quality improvement initiative.
