Dos grandes estudios clínicos en fase III en los que se comparaba la eficacia y la seguridad de fidaxomicina oral 400 mg/día con vancomicina oral 500 mg/día demostraron las ventajas de fidaxomicina sobre vancomicina, el tratamiento de referencia[11]. Los pacientes eran aptos para participar en estos ensayos clínicos si tenían un diagnóstico de ICD confirmado, incluidos los pacientes clasificados con ICD grave así como aquellos pacientes para quienes la ICD supone un riesgo especial, como los pacientes ancianos o con enfermedades concomitantes[12],[13],[14]. Fidaxomicina redujo significativamente la tasa de recurrencia de ICD en comparación con vancomicina; los pacientes tratados con fidaxomicina tenían una probabilidad significativamente mayor que los que recibían vancomicina de presentar resolución de la diarrea sin recurrencia en los 30 días posteriores a la finalización del tratamiento. Fidaxomicina también cumplió el criterio de valoración principal de tasa de curación clínica, que era equivalente al de vancomicina. Algunas poblaciones presentan un mayor riesgo de ICD y son especialmente vulnerables. La ICD puede estar asociada a un índice de mortalidad de hasta el 14 % en pacientes ancianos[15] y los pacientes oncológicos hospitalizados que están en tratamiento contra la ICD experimentan estancias hospitalarias más prolongadas[16].
