Las nuevas guías recomiendan fidaxomicina para todos los pacientes aptos para el tratamiento con antibióticos orales contra la infección por clostridium difficile

La infección por Clostridium difficile (ICD), una enfermedad potencialmente mortal, ha superado al MRSA como la causa principal de infecciones nosocomiales en los hospitales[1],[2]

La European Society of Clinical Microbiology and Infectious Diseases (ESCMID) ha publicado nuevas guías que recomiendan fidaxomicina para todos los pacientes con ICD aptos para el tratamiento con antibióticos orales. En concreto, las guías recomiendan fidaxomicina como tratamiento de primera línea en pacientes con ICD de primera aparición o en riesgo de enfermedad recurrente y en pacientes con múltiples recurrencias de ICD. Las guías también recomiendan fidaxomicina en pacientes con enfermedad grave e ICD no grave[3].

La ICD es una de las causas más frecuentes de diarrea asociada a antibióticos y los casos graves pueden llevar a la cirugía intestinal e incluso la muerte[4]. Los pacientes ingresados con ICD tienen una probabilidad hasta tres veces mayor de morir en el hospital (o en el mes posterior a la infección) que los que no presentan ICD[5],[6]. La recurrencia es un problema importante en el tratamiento de la ICD. El 25 % de los pacientes con ICD sufren una recurrencia al cabo de un mes[7],[8],[9] y los pacientes que ya han experimentado una recurrencia tienen un riesgo del 40 % de sufrir un nuevo episodio de ICD[10].
