Care Continuum Alliance (CCA), the leading voice in population health, is hosting the Showcase to provide a forum for population health and chronic disease management innovators to share expertise, technology, and strategies with lawmakers, regulatory and government officials, and others who work on expanding access to wellness, prevention and care management.
