Daten aus dem globalen Register zur Behandlung von Vorhofflimmern zeigen, dass antithrombotische Wirkstoffe zur Verhinderung von Schlaganfällen nicht optimal genutzt werden

  • AF-Patienten mit einem CHA2DS2-VASc-Risikoscore[6] von 0 haben gemäß den ESC-Leitlinien ein niedriges Schlaganfall-Risiko und sind keine Kandidaten für die VKA- oder NOAK-Antikoagulanzientherapie.[4]
  • Von den 440 GARFIELD-AF-Patienten mit einem CHA2DS2-VASc-Risikoscore von 0 erhielten 40 % ein Antikoagulans (32,4 % erhielten ein VKA und 7,6 % erhielten ein NOAK).
  • Des Weiteren bestätigten die Daten, dass diese Niedrigrisiko-Patienten tendenziell weniger unerwünschte klinische Ergebnisse erzielten, einschließlich eines wesentlich geringeren Sterberisikos jedweder Ursache im Vergleich zu Patienten mit einem CHA2DS2-VASc ≥1, was die Leitlinienempfehlung gegen die Behandlung von Niedrigrisiko-Patienten mit einer Antikoagulanzientherapie unterstützt.
  • Ältere Patienten haben größere Chancen eine Antikoagulanzientherapie zu erhalten[7]   

    • Bei den GARFIELD-AF-Patienten stand ein höheres Alter in Zusammenhang mit einer häufigeren Anwendung der oralen Antikoagulanzientherapie, einer erhöhten Inzidenz von Komorbiditäten und einem höheren Risiko von Schlaganfall-/systemischen Embolien, Tod oder schwerwiegenden Blutungsereignissen.
    • Bei Patienten im Alter von 65 Jahren oder darüber war der kardiovaskuläre Tod nicht die Haupttodesursache.
