ConnectiCare's Care Management Solutions Wins Contract with the State of Connecticut for Health Management

FARMINGTON, Conn., June 20, 2013 /PRNewswire/ -- Care Management Solutions, a ConnectiCare health plan affiliate company, has been contracted by the State of Connecticut as the new administrator for an innovative health management initiative known as the Health Enhancement Program (HEP). The HEP program was developed for Connecticut's state employees, recent retirees and their dependents and seeks to improve their individual health while working to reduce long term health costs for the State of Connecticut.

Over the next three years, Care Management Solutions will engage state employees, recent retirees and their enrolled family members to take charge of their health by providing them with health guidelines and prevention tactics. Care Management Solutions will also provide targeted, personal guidance to at-risk participants that could benefit from one of many health and chronic care management programs.
