Christiana Care Health System partners with ColdLight Solutions and Medecision for Bridging the Divides care model

WILMINGTON, Del., Feb. 26, 2013 /PRNewswire/ -- Christiana Care Health System has partnered with ColdLight and Medecision to provide more coordinated personalized care to patients with chronic heart disease with breakthrough analytics technology.

Christiana Care Health System has partnered with ColdLight Solutions, LLC and Medecision, Inc. in order to provide more coordinated care and greater value to patients with chronic heart disease.

The services of both companies will be folded into Christiana Care's new care model, known as "Bridging the Divides", which is designed to harness information technology to transcend the gaps that currently exist within health care. The Bridging the Divides program is supported by a $10 million award from the Center for Medicare & Medicaid Services.

Christiana Care was awarded $10 million for its model, which uses predictive analytics to identify patients who are more likely to experience readmissions, develop complications and need a higher level of care. The program also compares patient-specific outcomes with evidence-based recommendations in managing heart disease and uses a software-driven care management program to improve patient outcomes.
