Christiana Care Health System partners with ColdLight Solutions and Medecision for Bridging the Divides care model
Date:2/26/2013

WILMINGTON, Del., Feb. 26, 2013 /PRNewswire/ -- Christiana Care Health System has partnered with ColdLight and Medecision to provide more coordinated personalized care to patients with chronic heart disease with breakthrough analytics technology.

Christiana Care Health System has partnered with ColdLight Solutions, LLC and Medecision, Inc. in order to provide more coordinated care and greater value to patients with chronic heart disease.

The services of both companies will be folded into Christiana Care's new care model, known as "Bridging the Divides", which is designed to harness information technology to transcend the gaps that currently exist within health care. The Bridging the Divides program is supported by a $10 million award from the Center for Medicare & Medicaid Services.

Christiana Care was awarded $10 million for its model, which uses predictive analytics to identify patients who are more likely to experience readmissions, develop complications and need a higher level of care. The program also compares patient-specific outcomes with evidence-based recommendations in managing heart disease and uses a software-driven care management program to improve patient outcomes.

"As a CMMI grant recipient we have a responsibility to develop methods to improve the value of care delivery in ways that can be adopted by health care organizations across the country," said Terri Steinberg , M.D., Chief Medical Information Officer for Christiana Care. "These companies provide innovative technology solutions that can be adopted by large or small health systems to improve health care value anywhere in the country."

ColdLight (www.coldlight.com) delivers a big data-plying technology, known as Neuron™, which is powerful enough to tap into all available sources of data – ranging from clinical activity to lab results to pharmaceutical use – on each individual patient to unearth relevant predictive patterns and facts to improve patient care. Christiana Care will use the information provided through Neuron to ensure patients can safely transition from acute care settings to their homes for follow up care.

"Applying Neuron's machine intelligence, data science technology and big data pattern detection capabilities together with Christiana Care's clinical expertise and leadership, is a great opportunity to demonstrate a blueprint on how to truly transform the delivery of health care in our country," said Ryan Caplan , President and CEO of ColdLight, which is headquartered in Wayne, Pa.

Medecision also will play a key role in the Bridging the Divides model, through its care management technology platform, Aerial™, which will interface with Neuron through Aerial's strong, embedded interoperability framework. Aerial provides care coordinators with a comprehensive, up-to-date snapshot of each patient's health status. Care coordinators also will leverage the accredited clinical content and streamlined workflow within Aerial to manage transitions in care, customize treatment plans for individual patients and arm them with the information they need to take control of their own health.

"We are thrilled to deliver the platform required to power the information sharing and care coordination that the Christiana Care community, and the consumers they serve across Delaware and the greater Brandywine Valley, need to improve outcomes and control costs," said Deborah M. Gage , President and CEO, Medecision, also headquartered in Wayne, Pa.
