Hospital readmissions are estimated to cost the U.S. health care system $31 billion, according to an April 2009 study by the U.S. Agency for Healthcare Research and Quality. Twenty percent of all Medicare patients are estimated to return to the hospital within thirty days of discharge, and fifty-six percent will be readmitted within a year, according to a January 2009 study reported in the New England Journal of Medicine. To address these findings, CareCentrix has developed a structured program providing discharged patients with transitional, home-based support services. These services have proven to increase the likelihood of a successful recovery at home, avoiding a readmission to the hospital.
