Date:4/15/2013

EAST HARTFORD, Conn., April 15, 2013 /PRNewswire-iReach/ -- ActualMeds, a technology company providing solutions for team-based medication management at the patient point of care, is one of the 50 innovators selected from over 250 applicants who will debut with "The Hive" -the newest platform of TEDMED 2013 at the Kennedy Center for the Performing Arts Washington, DC, April 16-19, 2013. The Hive will bring together the leaders of today with the future leaders of tomorrow in a showcase of new approaches to innovation and scalable new ideas.

The Hive at TEDMED 2013 is also a debut for ActualMeds, formerly known as AdhereTx Corporation, who will feature its newest solutions to enable health care teams to conduct cost-effective medication reconciliation for high-risk patients along the care continuum. "We are delighted and honored to be chosen among this notable group of thought leaders and innovators" says Patricia Meisner , Chief Executive Officer and a co-founder at ActualMeds. "There has never been more environmental pressure than today for transformation in healthcare. With thought leaders and innovators immersed in discussion of healthcare's challenges, the Hive will be catalytic to change and identification of opportunities to improve the quality of healthcare. ActualMeds will be part of that transformation."

ActualMeds enables health care practitioners to access a patient's medication history when and where it is needed- from a primary care visit, to the emergency department, to home care.  Using standards-based interoperability, the solution incorporates "what the doctor ordered" from the patient's electronic health record, "what prescriptions got filled" from insurance claims data, and presents this validated information alongside of a structured interview to elicit "what the patient is actually doing". Evidence-based rules facilitate interpretation of risk
