Description Insulin-like growth factor binding protein 4 (IGFBP-4) is a 24 kDa protein which specifically binds and modulates the actions of IGF-I and IGF-II. It is also present in a glycoslyated form with a molecular weight of 28 kDa. IGFBP-4 has been identified in all biological fluids, as well as in numerous cell types, including fibroblasts, neuroblastoma, prostate and bone cells [1,2]. IGFBP-4 appears to be inhibitory to IGF action by binding the IGFs and preventing their interaction with the IGF receptors [1]. This inhibitory function of IGFBP-4 is protease dependent [2]. The regulatory mechanisms that control IGFBP-4 expression are still poorly understood, however in bone IGFBP-4 appears to be regulated by PTH and 1,25 dihydroxyvitamin D3 [2]. Serum levels of IGFBP-4 appear to increase with age. In elderly women, this increase appears to be due in part to the age-related increase in PTH production of which is stimulated in calcium deficiency states [3].
