| Description | Human Insulin-like growth factor binding protein-3 (IGFBP-3) is the main binding protein for IGF-I and IGF-II in postnatal serum, as a component of the ~150 kDa ternary complex. This complex consists of IGFBP-3, a molecule of IGF-I or IGF-II and ALS, allowing IGFBP-3, along with ALS, to regulate the actions of the IGFs. The molar concentration of IGFBP-3 in normal serum is proportional to the molar concentration of total IGFs (i.e. IGF-I + IGF-II) [1]. In addition, total IGF levels show higher correlation with GH in circulation than either IGF-I or IGF-II levels alone [2]. IGFBP-3 is GH-dependent, however IGFBP-3 may be less age-dependent than IGF-I [3]. These aspects of IGFBP-3 physiology allow its measurement to reflect GH activity. Thus, in GH deficiency of either pituitary or hypothalamic origin, IGFBP-3 levels are uniformly low. Blum et al. [3] reported that 128/132 children with GH deficiency had serum IGFBP-3 concentrations below the 5th percentile for age. In situations of GH receptor deficiency, such as Laron syndrome, anti-GH antibodies, liver disease or malnutrition, IGFBP-3 levels are also reduced, even in the presence of elevated serum GH concentrations [4]. Measurement of IGFBP-3 levels in serum is a useful diagnostic test for the clinical evaluation of GH-related disorders.
