| Description | Insulin-like growth factor II (IGF-II) is a 7.5 kDa peptide which is secreted primarily by the liver and is postulated to have mitogenic and metabolic actions involved in cellular growth. The actions of IGF-II are mediated by binding to target cell receptors. IGF-II binds with high affinity to the Type II IGF receptor, a monomeric protein which also serves as the receptor for mannose-6-phosphate [1]. However, it is believed that binding to the Type I IGF and insulin receptors may mediate the mitogenic actions of IGF-II [2,3]. In the circulation, the interaction of IGF-II with these receptors appears to be regulated by IGF-binding proteins (IGFBPs). IGF-II is mainly carried in blood in the high molecular weight ternary complex with IGFBP-3 and ALS, and a smaller proportion of IGF-II circulates in association with other IGFBPs [4,5]. Serum IGF-II concentrations increase with postnatal age showing a moderate age-related increase throughout childhood and puberty, and there is no significant variability during the day [6,7]. Serum levels of IGF-II are also dependent on GH secretion. IGF-II levels are low in GH deficiency [7], although IGF-II levels may also decrease in GH excess and with exogenous administration of IGF-I [3]. The combined molar concentrations of IGF-I and IGF-II (i.e. Total serum IGF) is proportional to serum IGFBP-3 and consequently may be useful for evaluation of GH-related disorders [8].
