Description The Immunoscan MPO-ANCA Kit is an enzyme immunoassay (EIA) kit for the in vitro detection in human serum of the subclass of Anti-Neutrophil Cytoplasmic Antibodies (ANCA) directed against myeloperoxidase to be used as an aid in the diagnosis of crescentic glomerulonephritis. ANCA are a family of autoantibodies associated with vasculitic disorders and glomerulonephritis (GN). They were first described in 1982 in patients with necrotizing GN (1) and in patients with vasculitis and GN in 1984 (2). In 1985 research on Wegeners granulomatosis (WG), independent from the earlier observations, described detection of ANCA (also called ACPA) as a diagnostic tool for Wegeners granulomatosis (3,4). In the efforts to identify the antigens for ANCA it was found that two staining patterns for ANCA could be described by indirect immunofluorescence (IIF) using ethanol fixed human neutrophils (5). One subclass of ANCA gave a finely granular, diffuse staining pattern of the cytoplasm of human neutrophils. At the 2nd International Workshop on ANCA, this subclass was termed c-ANCA for cytoplasmic staining or classical staining ANCA, and correlated to the ACPA associated with Wegeners granulomatosis (6,7). It had been shown that c-ANCA were directed against an antigen in the azurophil granules of the cytoplasm of human neutrophils (8) and later that the principal antigen was a 29kD serine protease identical to proteinase 3 or myeloblastin (9,10,11). When detected by antigen specific assays these autoantibodies are termed 29kD-ANCA or PR3-ANCA. The other subclass of ANCA yielded a perinuclear staining pattern. This was originally thought to represent granulocyte-specific anti-nuclear antibodies (GS-ANA) (5) but in sera from patients with GN such autoantibodies were shown to be due to ANCA directed against myeloperoxidase (MPO), also present in the azurophil granules in the cytoplasm of neutrophils (6,12). These antibodies were termed p-ANCA due to the perinuclear staining when detected by IIF and are termed MPO-ANCA when detected by antigen specific assays.
