What are the Qualifications for an Arizona Medical Marijuana Card?

Now detailed on Valley Evaluation Centers’ website, valleyevaluation.com, are the qualifications in order to receive a Medical Marijuana card. Their list of requirements begins with a trip to the doctor’s office. One must have a qualifying condition such as Cancer, Glaucoma, HIV, AIDS, Hepatitis C, ALS, or Crohn’s Disease; just to name a few. Second, one needs to submit one of three forms of identification (Arizona Driver’s License, Arizona State I.D. Card, or a U.S. Passport).

Next, one’s medical records must be reviewed by a physician licensed to practice medicine in the State of Arizona. There are numerous physicians throughout the state that are specialized in this regard. This process is considered to be just a confirmation that one qualifies. Lastly, the evaluating physician will complete a Medical Marijuana physician’s certification form. This, along with a signed Medical Marijuana patient attestation, a scanned copy of one’s I.D., and a scanned copy of a current photo; are what's needed to complete the process. These will be submitted (online) to the Arizona Department of Health services, and in ten business days; one receives their Arizona Medical Marijuana card.
