Wexford PHO Moves to SPH Analytics for Improved Population Health Management

SPH Analytics, a leader in healthcare analytics and population health management, today announced that Wexford Physician Hospital Organization (PHO) has successfully deployed SPH’s Population Care™ all-payer population health management solution to achieve its quality and cost goals within the framework of value-based care. Use of Population Care is benefitting more than 77,000 patients covered by value-based contracts across the 500-provider Physician Hospital Organization in Northern Michigan by streamlining care processes, monitoring quality, and helping providers identify and close gaps in care for improved patient outcomes.

Specifically, SPH Analytics and Wexford PHO are working to aggregate data across 30+ locations, incorporating eight EHR systems, as well as data from claims, appointment schedules and other healthcare sources. Specifically, SPH Analytics is helping Wexford PHO:
