Patient Registry software captures and manages clinical information on diseases, conditions and patient populations to support evidence-based medicine for preventive care and chronic disease management.

Patient Registry provides advantages to physicians and clinics adopting the 'medical home' model of preventive health care. Studies in the New England Journal of Medicine and other publications show that the 'medical home' model is gaining ground as an important step toward the goal of better patient outcomes and reduced medical costs. With this same goal in mind, the U.S. government (HHS-Medicare, Medicaid, etc.) is considering the medical home model for population health as a foundation for restructuring the current provider payment system.

The medical home uses networked and Internet-connected PCs to provide a centralized point of care management for patients and the coordination of their treatment. In this environment, the Patient Registry application identifies patients overdue for checkups or treatments, provides clinical guidelines, generates automated physician alerts and patient reminders, assesses improvement in quality of care, and produces provider-specific reports to monitor gaps in care. This proactive solution helps physicians improve management of basic care, specialist referrals, or hospitalization.

Patient Registry can be used as a stand-alone database application, or integrated with the VersaForm Electronic Medical Record (EMR) or Practice Management systems. No special expertise or training is required to implement Patient Registry, which runs on most Microsoft Windows PCs. In addition, Medicare (PQRI) and other Pay-for-Performance (P4P) programs now provide financial incentives to use an automated system of alerts and reminders in practice management.

Alerts to Caregivers
Patient Registry alerts physicians to patient-specific suggestions, screening tests, exams, and other preventive services at the point of care. The conditions for displaying alerts may be set or modified by the provider, to follow established guidelines or other requirements. Alerts can be individualized to address a patient's specific clinical situation. The system tracks the preventive or disease management services provided, or overridden, and documents the reasons.

Reminders to Patients
Patient Registry helps physician groups to create, update, and individualize the rules for patient reminders. Between healthcare visits, patients receive automatic notifications of preventive services, tests, or behavioral actions that are due or overdue. The system sends notifications by letter or email and records the fact, and it can generate a printed list of patients that prefer to be notified by telephone.

Individualized Visit Plans
Visit Plans bring relevant patient information together with guidelines for managing each patient's conditions at the point of care. The Visit Plan includes a summary of the patient's key clinical indicators (medications, test results), with suggestions to be presented. If the computer system is not used at the point of care, Visit Plans can be printed in advance, including alerts. Visit Plans can be printed for one or all of the patients scheduled for a particular day or for a period of time over the course of treatment.

Prepared and Ad-hoc Patient Care Reports
Patient Registry ensures easy access to relevant reports so physicians can track patients who need special attention. These reports measure changes in the practice's performance and results against clinical guidelines to identify gaps in care. Physicians can search on an individual patient or aggregate information on patients with a specific disease or condition. Reporting on predefined populations includes common conditions such as diabetes, CAD and CHF with coded definitions endorsed by the AMA, CMA or NQF.
