VNA of Hanover & Spring Grove Selects Cardiocom Telehealth Technology as Its New Telehealth Provider

MINNEAPOLIS, Aug. 31 /PRNewswire/ -- VNA of Hanover & Spring Grove, who recently celebrated its 100th anniversary, announced that it has entered into a business relationship with Cardiocom(R) to provide advanced home telehealth products and services to patients with chronic diseases. Cardiocom, the Experts in Telehealth(SM), is world class developer, manufacturer and clinical service provider of award-winning telehealth solutions.

Cardiocom's telehealth system provides a unique opportunity to proactively manage and customize patient care. Every day, VNA of Hanover & Spring Grove patients use the Cardiocom Commander Telehealth System to communicate with a VNA telehealth nurse. The Commander Home Telemonitoring System guides the patient through an interactive Health Check and collection of vital signs including: weight, blood pressure, oxygen saturation and heart rate. This information is immediately available for review via a standard phone line to the agency's telehealth department.

"The Cardiocom System is a key tool that proactively ensures coordinated care from a hospital, nursing home or physician's office to the patient's home," says Joni Griffin, RN, BSN, Chief Clinical Officer for the VNA. "By transmitting objective, real-time health data every day, the patient is connected to our staff so that their care plan can be continually updated as needed. This system uses today's most advanced home care technology and monitoring services.
