"Patient compliance or adherence, often looked to as medication-related outcome measure, is not the primary issue related to the inappropriate use of medication. With better selection and monitoring, adherence would be very much a secondary issue, mostly related to the cost of medications, not their side-effects. Pharmacist-driven medication management helps patients avoid medication problems by assuring the appropriate use of medication, prior to taking them, and through ongoing patient monitoring in partnership with the patient, prescriber and other care providers."

"Automating the dispensing functions of pharmacy is a key strategy that enables clinically trained pharmacists to work with other healthcare providers to focus on patients, and not pills," said Jim Goers, RPh, also a member of the TR-Strategies team. "Pharmacy automation reduces the risk of human error in the dispensing process and increases resources available for clinical functions, but does not assure that the medication itself is appropriate for the patient. This is the role of the pharmaceutical care practicing pharmacist -- assuring appropriate medication selection."

The most recent study of the cost of medication-related morbidity and mortality found that $177 billion was spent in the year 2000 to treat the problems associated with medication, which is more than was spent on the cost of medications themselves, according to the Cost of Illness Model, published in the Journal of the American Pharmacists Association, 2001, by Ernst and Grizzle. Numerous studies have been published that underscore the benefits of clinical pharmacist-driven pharmaceutical care including reduced overall costs of healthcare, fewer emergency room and physician visits, and initial or extended hospitalizations. American Pharmacist Foundation Projects such as Project ImPACT, the Asheville Project, the Patient Self-Management Program for Diabetes and the Diabetes 10 City Challenge have found improved patient
