U.S. Care Management, formerly known as Specialty Disease Management, was organized in 1997 by health professionals who recognized the value in coordinating care to serve people with chronic diseases. Based in Florida and contracted in six other states, U.S. Care Management's experience offers numerous assets that are recognized for their unique contribution to disease management programs.
