Seamless Medical Systems Announces the Launch of SNAP Express Rx™ to Enhance Pharmacy Patient Engagement and Clinical Care Coordination
Date:8/25/2014

Santa Fe, NM (PRWEB) August 25, 2014

SNAP Express Rx™ uses an iPad to replace the clipboards and paper forms patients fill out for procedures such as: Flu vaccines, immunizations, medication therapy management (MTM), comprehensive medication reconciliation (CMR), and other clinical services. SNAP Express Rx™ is a cloud-based enterprise solution with bi-directional data exchange with electronic medical records systems that eliminates paper, printing, scanning, faxing, shredding and data entry. SNAP Express Rx™ improves workflow efficiency, reduces costs and waste, enhances data quality, reduces denied claims and improves patient satisfaction and loyalty.

According to the founder and CEO of Seamless Medical Systems, David J. Perez, “As pharmacies and in-store clinics expand their reach and services, the need for improved workflow efficiency and care coordination becomes more critically important. It’s 2014, yet the standard data capture device is quill and parchment. SNAP Express Rx™ is a 21st century solution designed to expand patient throughput while improving the customer experience. Our platform supports pharmacy care coordination at the highest levels, making it possible to track and coordinate Pharmacy Quality STAR measures required by CMS starting in 2015.”

SNAP Express Rx™ also seamlessly connects patients to a mobile health education platform in the store called MyHeal+H™, which provides patients with engaging, trusted and accurate health information personalized to the individual patient’s age, gender and reason for visit. The educational material presented includes content from the Mayo Clinic, National Institutes of Health and Prevention Magazine.

MyHeal+H™ is also an in-store marketing platform serving customers targeted, relevant offers and promotions.
