Sangre de Cristo Hospice & Palliative Care Expands Usage of Mobile Healthcare Solution with CellTrak

Sangre de Cristo Hospice & Palliative Care has long set the standard for end-of-life care in Southern Colorado. They continue to address the individual needs of the patients and families they are privileged to serve. As a nonprofit, Sangre de Cristo Hospice & Palliative Care serves everyone who needs them, regardless of ability to pay. They offer grief support to anyone who has suffered a loss, whether or not their loved one was a patient. And they provide educational offerings on all matters related to death and dying throughout the area. Since the beginning of their work in Colorado they have made it their mission to preserve the dignity of their patients and provide them with a world-class level of care.

The mission of Sangre de Cristo Hospice & Palliative Care is to provide terminally ill patients with the necessary physical, emotional and spiritual support to ensure the best quality of their remaining lives. This allows for them to die with dignity and also to provide patient’s families and friends with emotional support for the remainder of the patients’ lives and thereafter. Implementing a technology solution throughout the office allows for agencies to spend less time on paper and more time on care. They have adapted to multiple technology programs with their first type being their EMR, Suncoast Solutions, several years ago. With their mission in mind to create improved streamlined processes and workflow for better patient care, mobile technology was an easy decision to simplify processes and help reduce costs. They chose to deliver services at the point of care via CellTrak on iPhone devices on the Sprint network.
