Pulmonary Horizons 2nd Annual COPD Conference Miami Hilton Downtown, July 17-19, 2015 Patient Centered Cross-Continuum Education to Improve the Care of Patients with COPD

This year the National Lung Health Education Program (NLHEP) partners with Pulmonary Horizons to offer one of the most diverse perspectives with speakers and attendees to effectively identify and discuss successful practices to diagnose, treat and manage patients with COPD.

"The Value of Attending the Pulmonary Horizons Conference is Interaction with Multidisciplinary Healthcare Professionals who represent various Perspectives that can address the Overall Needs of Patients with COPD," said Brian W. Carlin, MD, FCCP, FAARC, Chairman of the National Lung Health Education Program.

NLHEP has collaborated with the Pulmonary Horizons Program Committee on the development of the pre-conference Spirometry Course and COPD Coordinator Course, offered on Friday July 17, 2015.

"This year's conference kicks off with a Keynote Presentation by Nicholas “Dr. Nick” Yphantides, MD, MPH, Chief Medical Officer, Health & Human Services, San Diego County," said Vernon Pertelle, MBA, RRT, Pulmonary Horizons program committee member and associate executive director.

Dr. Nick currently serves as the Chief Medical Officer for San Diego County. He was the Chief Medical Officer (CMO) of one the largest network of Community Clinics in San Diego County, the Council of Community Clinics and was the publicly elected Chairman of the Board of Directors for Palomar Health, the largest Public Hospital District in California. As a result of his personal health transformation he now advocates for population and regional health transformation and is passionate about putting the health back into healthcare and keeping the patient at the center of their care.

The panel discussions that follow include a focus on new care delivery models as well as innovative approaches that have been developed and implemented by various organizations. The panels on Saturday July18th include:

Patient Perspectives of Care; Accountable Care Organizations (ACOs) - Program Development and Implementation; Patient Centered Medical Home (PCMH) - Primary Care across the Continuum; Retail Health Clinics Role in Primary Care; Financial Considerations, Challenges and Opportunities; Community-Based Pulmonary Rehabilitation Opportunities; Medical Device Management and Compliance; Pharmaceutical Management and Compliance; and the day is capped off by an Evening Reception & Fundraiser hosted by Lauren Evans, RSI NJ specialists in silent auctions.

The theme for Sunday July19th is associated with Innovation, New Technology, Telehealth and Remote Technologies. The Keynote Presentation will be provided by Amritha Kidiyoor, MS, PhD, Researcher, Wake Forest Baptist Medical Center, Institute for Regenerative Medicine, Winston Salem, NC. Dr. Kidiyoor currently serves as a researcher under Anthony Atala, MD, Director at the Wake Forest Baptist Medical Center's Institute for Regenerative Medicine. Her specific interests are associated with reprogramming fibrotic lung tissue to functional epithelial lung tissue. She works with more than 300 scientists in the fields of biomedical and chemical engineering, cell and molecular biology, biochemistry, pharmacology, physiology, materials science, nanotechnology, genomics, proteomics, surgery and medicine to translate the science of regenerative medicine into clinical therapies, that will benefit patients with chronic lung disease.

The panel discussions includes: Innovation and Emerging Technology; Telehealth & Remote Technologies; Program Development and Implementation; New Technology vs. New Products; Roundtable Discussion: Clinical Research in Non-Traditional Settings; and Integrating Home Medical Equipment (HME) Providers in the Continuum of Care.
