TransforMED Names Kevin Hutchinson to Inaugural Board of Managers
BETHESDA, Md., May 27 /PRNewswire/ -- Prematics president and chief executive officer, Kevin Hutchinson, has been named an inaugural member of the Board of Managers for TransforMED LLC, a wholly owned subsidiary of the American Academy of Family Physicians (AAFP). TransforMED will provide support and consulting services to primary care physicians and health systems that want to transform their medical practices into patient-centered medical homes.
The TransforMED medical home model combines the relationship-centered values of family medicine with new technologies to improve care quality, efficiency and patient and physician satisfaction. As patient-centered medical homes, primary care practices serve as the focal point of care coordination across all specialties for patients, maintaining central and complete patient records. The model also calls for adoption of information technologies as an essential component to improving diagnostic capabilities and enhancing treatment and outcomes.
"The complex challenges facing today's health care system are forcing physician practices and motivating patients to rethink the way care is delivered," said Mr. Hutchinson. "I'm pleased to be a member of the TransforMED board as it provides critical tools and support for practices navigating the transformation to a patient-centered medical home and implementing new technologies for more coordinated, patient-centered care."
Mr. Hutchinson leads Prematics, a company assisting practices with the move to more connected, efficient care through the exchange of clinical transactions and patient information using standards-based and proven technologies, starting with electronic prescribing. The company's medication management service, ScriptTone, helps practices improve transparency between physicians through access to more complete, up-to-date prescription information. Providers can view a patient's past and current record of prescriptions written by the home practice and outside practices, check for drug interactions and lower cost alternatives, and electronically transmit prescriptions to pharmacies.
"We are delighted to have Kevin Hutchinson on the TransforMED board," said Terry McGeeney, president and CEO of TransforMED LLC. "Helping physicians leverage technology and manage change is an integral part of the transformation process. Kevin's experience developing and deploying systems that address today's most pressing health care challenges will prove incredibly valuable to helping TransforMED fulfill its mission."
Prematics is an innovative provider of clinical IT services for physicians that improve the safety, quality and efficiency of health care. The company offers ScriptTone, the nation's first easy-to-use, fully-managed electronic prescribing service designed for today's busy practices. This reliable and highly secure service is delivered over the company's proprietary end-to-end network and equips physicians with the critical patient information they need at the point-of-care to prescribe the most clinically appropriate and cost-effective medications. ScriptTone benefits all of the key industry stakeholders by driving patient safety, lowering costs, and streamlining fulfillment. TransforMED is focused on practice redesign and affiliated with the
American Academy of Family Physicians (AAFP). TransforMED is studying and
implementing transformed models of high performance practices that meet the
needs of both patients and practices. In June 2006, TransforMED launched a
24-month National Demonstration Project (NDP), serving as a "learning lab"
to generate new knowledge about the process of practice transformation and
to systematically evaluate and compare the effect of two practice
transformation approaches on practice and patient outcomes. As results and
insights emerge, TransforMED professional staff are using the lessons
learned from the NDP to develop services, collaboration tools and learning
opportunities that empower physicians and primary care practices across the
country as they implement the TransforMED Medical Home. TransforMED also
coordinates a residency demonstration initiative known as P4 -- P to the
fourth power -- which stands for Preparing the Personal Physician for
Practice. The P4 residency demonstration initiative evaluates and supports
innovations in family medicine residencies. To learn more about TransforMED
