The TransforMED medical home model combines the relationship-centered values of family medicine with new technologies to improve care quality, efficiency and patient and physician satisfaction. As patient-centered medical homes, primary care practices serve as the focal point of care coordination across all specialties for patients, maintaining central and complete patient records. The model also calls for adoption of information technologies as an essential component to improving diagnostic capabilities and enhancing treatment and outcomes.
"The complex challenges facing today's health care system are forcing physician practices and motivating patients to rethink the way care is delivered," said Mr. Hutchinson. "I'm pleased to be a member of the TransforMED board as it provides critical tools and support for practices navigating the transformation to a patient-centered medical home and implementing new technologies for more coordinated, patient-centered care."
