Portable Health Care Records Now Available for Consumers
Date:8/5/2009

OAKLAND, Calif., Aug. 5 /PRNewswire/ -- Anyone can now easily create a personal and portable health care record in a standardized format understood at hospitals, clinics and doctors' offices. Heartwood Health, a health care management company specializing in chronic illness support, geriatric care management and private health care advocacy, has announced a partnership agreement with Chart Your Own Health to re-brand and sell the popular My Health Care Record medical documentation system.

"We can't wait for a universal health care record to arrive. People need this technology today," says Heartwood Health CEO, Michelle Gilmore. "This product brings our clients into a standardized medical communication system with their caregivers and providers, is very low cost, and takes just hours to complete," she continued.

Heartwood Health Private Health Record is a comprehensive health records organizer not unlike the familiar and easy to use time management organizers that have been popular in business for decades. The organizer is divided into tabs and forms and can even track personal data over time, such as weight or blood pressure readings. Because it uses the paper organizer paradigm - familiar to almost everyone - it is "elder friendly," and requires no computers or complicated software, which can be so hard to learn for many of the elderly.

"If you can mark up your calendar, you can keep an excellent, well organized medical record that is very helpful to you and your caregivers," claims Heartwood's Gilmore. "This product has the great potential to eliminate medical errors too," she concluded.

Heartwood will begin selling the Heartwood Health Private Health Record system on their website www.heartwoodhealth.com as of August 5th. The price is $29.95.

Heartwood Health, Inc. was founded in 2004 and is located in Oakland, CA. They offer private healthcare advocacy services in addition to their specialty pharmacy division providing home care pharmacy services for patients with chronic disorders like Hemophilia.

Chart Your Own Health was founded in 2006 by Amy Sluss, RN. Ms. Sluss developed the My Health Care Record system to go with her book, Health Care your Way - How to Get the Best Patient Care. Ms. Sluss writes a popular blog and offers workshops. Her web site is: www.chartyourownhealth.com.
