Porchlight VNA/Home Care Adopts HRS Telehealth to Improve Patient Outcomes
Date:11/8/2018

Porchlight VNA/Home Care has adopted telehealth software from Health Recovery Solutions (HRS) to reduce re-hospitalizations and enhance disease management for high risk patients.

Porchlight VNA/Home Care plans to utilize the HRS software to augment care for patients who are at risk for hospital readmissions and emergency department visits, primarily patients with CHF and COPD. The team at Porchlight VNA/Home Care made the decision to partner with HRS due to the state-of-the-art technology that can improve clinical workflows and subsequently result in better patient outcomes.

Patients placed on Porchlight VNA/Home Care’s telehealth program are provided with 4G tablets pre-loaded with the HRS software. The tablets pair with disease specific Bluetooth biometric devices that allow patients to track their blood pressure, weight, heart rate, and more. The patient data automatically transmits to the tablet and is electronically delivered to the patient’s nurse, allowing clinicians to quickly address any changes in medical status.

The HRS telehealth software also facilitates patient participation and self-management with daily medication reminders and disease specific educational content. Additionally, patients can actively communicate with their clinicians in real time using the tablet’s text, phone call, and video chat features.

The clinicians at Porchlight VNA/Home Care are particularly excited about their ability to use the HRS software to video conference with patients and capture wound images. These advanced software features will allow the team at Porchlight VNA/Home care to maintain a high level of care remotely and prevent unnecessary hospitalizations.

Kathleen Stezko MSN, RN, Vice President of Clinical Services at Porchlight VNA/Home care, states, “There are so many benefits to the patient receiving telehealth in the home. The benefits include: reduction in the cycle of emergency room visits and hospital readmissions; increased patient compliance; early detection and intervention; clear, concise, and accurate patient data in a timely manner; as well as patient satisfaction and increased confidence with the patient’s ability to remain home and manage their chronic illness.”

Jarrett Bauer, CEO of HRS, adds, “Porchlight VNA/Home Care understands the role of remote patient monitoring in improving patient outcomes and enhancing healthcare efficiency. We are honored to have been chosen as Porchlight VNA/Home Care’s telehealth partner and we are eager to see how their program unfolds.”
