Performance Clinical Systems Streamlines Clinical Workflows for Holston Medical Group in Their Value-Based Agreements

Performance Clinical Systems announced today at the Health 2.0 Conference the commercial availability of the latest release their cloud-based groupware application for clinical workflows, Symphony 4.0. One of the company’s key clients, Holston Medical Group (HMG) in Kingsport, Tennessee, implemented the Symphony update as an essential component of their Accountable Care Organization's (ACO) quality and performance strategies.

Symphony 4.0 works in unison with traditional EMRs and other enterprise HIT systems, but offers a distinctly different and incremental set of functionality not found in any of those systems. Using a patented "dynamic checklist" system, Symphony provides decision support and performance documentation for virtually any patient encounter. HMG is using the platform for transitions in care handoffs from inpatient hospitalists to primary care practices to better manage post-hospitalization care plans. They are also using Symphony across all clinical settings to achieve and document 29 of the 33 quality and performance measures required in the Medicare Shared Savings Program.

Vital information about patient's attitude regarding their condition, their belief on how well they can manage a condition, their home environment, caregiver situation, and any behavioral factors are all documented within Symphony and made available throughout the clinical workflow.
