Pathways Home Health & Hospice Receives 2012 HomeCare Elite Top Honor for Quality Care

Pathways Home Health & Hospice received top honors from the National Research Corporation, an organization that tracks quality of care metrics within the home health industry. The 2012 HomeCare Elite designation is given by the National Research Corporation to home health agencies that excel in quality patient care.
The HomeCare Elite identifies Medicare-certified agencies and highlights the top agencies overall. Winners are ranked by an analysis of publicly available performance measures in quality outcomes, process measure implementation, and patient quality improvement.
“Receiving the HomeCare Elite designation is a great honor,” said Georgia Rock, Pathways Chief Operating Officer. “Providing compassionate, high quality patient care is our mission, and the 2012 HomeCare Elite recognition is evidence that we are meeting the needs of our community.”
Pathways Home Health & Hospice is the one of the oldest home health and hospice organizations in the San Francisco Bay Area, having served patients in their homes for more than 35 years. A non-profit healthcare agency, Pathways is affiliated with El Camino and Sequoia Hospitals. Pathways is also accredited by The Joint Commission for the accreditation for healthcare organizations, an independent organization that endorses only those hospitals and home health agencies that meet the strictest standards.
