PDHI Adds Diabetes and Heart Disease Prevention to ConXus Steps Library of Action Plans

All the health action plans can be configured to match client service levels and business rules, including program duration, action steps included, and incentive points available. Integration with ConXus Rewards provides comprehensive tracking and fulfillment of incentives.

ConXus Steps action plans are an integral component of the ConXus platform. Designed for organizations that deliver health and wellness programs to multiple employer end clients, the platform enables assessment and stratification, biometric screening data integration, behavior change reporting, health coaching, wellness challenges and events, and incentive tracking and fulfillment.

ConXus Steps also supports online wellness programs in the form of:

  1. Challenges - Health trackers that are delivered company-wide or to selected communities to build team-based programs
  2. Events - Employer-defined health activities that occur throughout the year, such as wellness seminars or flu shots. Employers can update activities and related incentives using a secure employer portal.

ConXus Steps supports a self-directed model for personal health improvement or a higher-touch, coach-directed model when combined with ConXus Direct. Using ConXus Direct, health coaches assign plans to participants, review their results, and record steps, such as coaching sessions or biometric results.

Client administrators are provided with a robust set of online tools for managing projects and reporting data.
