Norwell VNA and Hospice Selects Cardiocom Telehealth as Its New Technology Vendor

MINNEAPOLIS, July 21 /PRNewswire/ -- Norwell VNA and Hospice, a 2008 HomeCare Elite top agency, announced that it has entered into a business relationship with Cardiocom((R)) to provide advanced home telehealth and disease management technology to patients with chronic illness. Cardiocom, the Experts in Telehealth(SM), is a world class developer, manufacturer and clinical service provider of award-winning telehealth solutions.

Cardiocom's telehealth system provides a novel interactive opportunity to proactively manage patient care. Every day, Norwell VNA and Hospice patients use the Cardiocom Commander Telehealth System to communicate with a Norwell VNA and Hospice nurse. The Commander Home Telemonitoring System guides the patient through an interactive symptom management assessment and collection of vital signs including: weight, blood pressure, oxygen saturation and heart rate. This information is immediately transmitted via a standard phone line or cellular connection to the patient's caregiver for review.
