CHICAGO, Sept. 22 /PRNewswire-USNewswire/ -- Lake Forest Hospital and Northwestern Memorial HealthCare announced today that they have signed an affiliation agreement to form an integrated healthcare system. Lake Forest Hospital will become a subsidiary of Northwestern Memorial HealthCare, the corporate parent of Northwestern Memorial Hospital.

Northwestern Memorial and Lake Forest Hospital expect to close the affiliation following receipt of the necessary federal and state approvals. This process could take several months.
