National Transitions of Care Coalition to Host Regional Summit in Cleveland

The National Transitions of Care Coalition (NTOCC), an organization focused on addressing and improving patient care transitions in the U.S. health care system, is set to host the Cleveland Regional Transitions of Care (TOC) Summit this fall to raise awareness about the challenges of poor transitions and present solutions.

This event will bring together health care professionals, patient and caregiver advocates, and policy makers with NTOCC thought leaders and partner representatives to address a serious U.S. health care issue: filling the gaps that occur when patients leave one care setting and move to another care setting.

With a dynamic program tailored to the specific needs of the community, attendees will discover barriers to improved transitions, effective solutions and emerging resources in development by area organizations. Additionally, the Cleveland event will provide avenues for local stakeholders to collaborate in the development and advancement of transitions of care initiatives.

This year’s agenda includes a full day of sessions, encompassing a regional TOC expert panel, an overview of NTOCC’s work and resources, sessions on healthcare reform and technology as they relate to TOC, and collaborative breakout sessions. Cheri Lattimer, RN, BSN, NTOCC Executive Director will speak and present at the event, as well as area thought leaders such as Mary McLaughlin Davis, RN, ACNS-BC, CCM, Director of Care Management, Lakewood Hospital; Marilyn A. Culley, RN, MSN, Vice President of Clinical Operations, ExactCare Pharmacy; Dr. H. Edward Davidson, PharmD, MPH, Asst. Professor of Clinical Internal Medicine, Eastern Virginia Medical; Will Hicklen, CEO, Ankota; and Matthew Tanzer, Chief Commercial Officer, RightCare Solutions.
