National Transitions of Care Coalition Adds Healthcare IT Innovations Task Force Member

The National Transitions of Care Coalition (NTOCC), an organization dedicated to improving patient transitions throughout the healthcare system, has added a member to its recently established Healthcare IT Innovations Task Force. Vree Health, a provider of technologies that meet the needs of health care providers, payers, and consumers, is the third member of the newly formed group. The Health IT Innovations Task Force initiative and its two founding members, RightCare Solutions and Ankota, were announced in October 2013.

Cheri Lattimer, RN, BSN, NTOCC Executive Director, remarked, “We are pleased to welcome a third member to this exciting Task Force. Managing transitions of care requires new and innovative technologies, and Vree Health, along with our two founding members, combine to create a powerful and innovative team with which NTOCC can partner to consider transitions of care innovations and improvements.”

In bringing together healthcare technology thought leaders who offer new, innovative or emerging solutions for care transitions challenges, NTOCC and Task Force members work toward the shared vision of advancing transitions of care (TOC) issues and improving patient outcomes and care quality. This complementary and diverse group utilizes their expertise in creating TOC solutions to collaborate with the NTOCC Board of Directors and Health Information Technology (HIT) Workgroup, identifying existing gaps in HIT and considering solutions to help eliminate barriers to improved transitions.
