IN Compass Health's Hospitalist Team at Morton Plant wins first BayCare Quality Award

re upon discharge from the hospital. Through a special arrangement with the Turley Clinic, the IN Compass Hospitalists encourages these patients to receive a follow-up outpatient visit for clinical evaluation, diabetic education and medications, at low or no cost to the patient.

Hundreds of posters and presentations were delivered on quality projects. Dr. Messler team's project was ultimately selected for meeting all the stringent requirements for the project. Dr. Messler states "we are very encouraged by the initial results of this project." Of the diabetic indigent patients discharged by his Hospitalist team, results have shown:

 
  • 93% have followed through on at least one outpatient office visit
  • 34% reduction in 30-day readmission rates
  • 3% reduction at A1c at three months

Denise Remus, PhD, RN and Chief Quality Officer at BayCare Health System had this to say about the project. "The award acknowledges the Hospitalist Group's amazing commitment to improving quality of care, care coordination, and outcomes for these patients. We are hopeful the work can serve as a model for similar programs."
