Health Services Authority Case Study Demonstrates the Efficacy of “People-Centered” Primary Care in Cayman Islands
Date:9/13/2017

Health Services Authority (HSA) recently published a case study evaluating the results of their journey towards People-Centered Primary Care in the Cayman Islands. As defined by World Health Organization (WHO), the term ‘People-Centered Health Care’ is not to be confused with patient-centered health care. People-Centered Health Care is an umbrella term which better encapsulates the foremost consideration of the patient across all levels of healthcare.

This journey at HSA resulted in numerous improvements in outcomes over the past year like a 60% increase in fall screenings, a 20-30% decrease in avoidable ER visits, and a 22% reduction in asthma and COPD admissions. HSA has many of the same population health challenges that healthcare organizations in the US, UK, and countries in Europe face. Patients are living more sedentary lifestyles and obesity, hypertension, and diabetes are more common now than ever.

“The Mission of the Health Services Authority is to provide the highest quality healthcare and improve the wellbeing of people in the Cayman Islands through accessible, sustainable, patient-focused services by highly-skilled, empowered, and caring staff in collaboration with our partners,” says Lizzette Yearwood, CEO at HSA.

HSA believes that the best way to improve outcomes and lower cost is to incentivize their physicians to keep their patient population healthy, a strategy they call People-Centered Primary Care. With this strategy in mind, they developed The Quality Care Initiative, a group of core quality-based objectives that became the primary focus of HSA.

“It was clear that improved clinical quality is what motivates clinicians,” said Keith Higgins, Chief Information Officer at HSA. “Healthcare must move away from just doing procedures to increase revenue; the focus must be on keeping the patient well. Regular screening is necessary so that patients don’t come to the emergency room unnecessarily, making sure they are on the proper medication if they are chronically ill, and monitoring the chronically ill patient from time to time to prevent admissions that will lead to increased cost. This will be of better value to both the patients and the provider.”

This transition from performance-based care to value-based care required significant assistance from VitreosHealth. Using Vitreos’ predictive and prescriptive analytics, HSA's physicians can monitor their patients’ gaps-in-care efficiently. VitreosHealth offers this through an Insights-as-a-Service (IaaS) delivery model that uses proprietary algorithms and AI-driven analytics to handpick high risk patients for high-touch care management before they suffer a major health event. Each “insights-view” achieves a patient-centric focus from every stakeholder’s perspective within the healthcare organization that will contribute to quality health outcomes and improve financial performance.

“The success of the People-Centered Primary Care program in the Cayman Islands can serve as a blueprint for population health programs in the North American, Caribbean, European, and Asian countries who are facing the daunting task of providing better healthcare at lower costs” says Jay Reddy, CEO of VitreosHealth.
