Health Integrated to Showcase Synergy Personal Health Management(R) at DMAA Conference

TAMPA, Fla., Sept. 12 /PRNewswire/ -- Health Integrated, Inc., a pioneer in health and condition (disease) management, today announced that it will be highlighting its ground-breaking Synergy Personal Health Management program at DMAA's Disease Management Leadership Forum and Integrated Care Summit beginning September 16th at Caesar's Palace in Las Vegas.

With the rapid evolution of disease management comes a need to stay informed of the best practices, latest technologies and most innovative programs today. The DMAA conference enables the field's best and brightest to convene on the latest developments on critical topics, policy issues and disease management models that change the way health care is delivered to chronically ill patients worldwide.
