Grane Home Health Care was looking for a cost effective point of care solution to interface with their existing McKesson Horizon system to assist with delivering patient specific care plans at the point of care. Working with Grane, CellTrak was able to create a seamless interface to provide a connection between the field staff and office staff for streamlined automation, reduced reimbursement and a data flow tool to access information in real-time. Face sheet notes flow from the mobile device to the office allowing for better care in real-time.
This secured information exchange improves the patient overall point of care experience. Grane chose a combination of devices for their mobile field staff including the iPhone and Blackberry running CellTrak on the Sprint and AT&T networks. Along with their multiple devices, they are now using multiple disciplines including RN’s Chaplains, CNAs, and Social workers all on the CellTrak solution for the Grane Hospice division.
