Grane Home Health Care and Grane Hospice Connects to Improved Care with CellTrak
Date:2/28/2013

Schaumburg, IL (PRWEB) February 28, 2013

Grane Home Health and Grane Hospice prides itself on helping patients achieve their highest level of functioning after illness or injury so that they may continue to live comfortably in their home environments and be restored to their optimal health. Grane Home Health Care strives to offer a balance of traditional and innovative services to provide loved ones timely, safe and effective patient-centered home care services. Family caring for family — Grane Home Health Care is locally owned and is committed to maintaining the highest level of care and rehabilitative services to meet the needs of the community.

Grane Home Health Care was looking for a cost effective point of care solution to interface with their existing McKesson Horizon system to assist with delivering patient specific care plans at the point of care. Working with Grane, CellTrak was able to create a seamless interface to provide a connection between the field staff and office staff for streamlined automation, reduced reimbursement and a data flow tool to access information in real-time. Face sheet notes flow from the mobile device to the office allowing for better care in real-time.

This secured information exchange improves the patient overall point of care experience. Grane chose a combination of devices for their mobile field staff including the iPhone and Blackberry running CellTrak on the Sprint and AT&T networks. Along with their multiple devices, they are now using multiple disciplines including RN’s Chaplains, CNAs, and Social workers all on the CellTrak solution for the Grane Hospice division.

William Gallagher Chief Operating Officer (COO) at Grane Home Health Care and Grane Hospice, shared, “Our team has created a vision that CellTrak is helping enable. By using CellTrak integrated with the McKesson Horizon solution our field staff is more connected and better informed as they deliver home healthcare.”

Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. Their aides provide additional personal care through bathing, feeding and dressing assistance. Their volunteers are also available for visits, errands and companionship.

“We are very pleased to help Grane Home Health Care and Grane Hospice achieve their stated goals of improved care and staff efficiency with our VisitManager solution,” shared Michael K. Wons, President and CEO of CellTrak Technologies, “The technology and care teams at Grane are true industry leaders and early adopters of these powerful new, yet easy to use technologies”.

VisitManager assists Grane Home Care and Grane Hospice with care plan delivery and complete office staff automation. CellTrak solutions provide an easy to use web portal and integrated mobile application that runs on GPS enabled devices or tablets across any carrier network. Streamline workflow, increase productivity, and improve patient experience at the point of care. This solution was able to meet Grane Home Health Cares operational needs as well as their overall outcomes for patient care.
