GSI Health Technology Provides Comprehensive Care Management to Health Plan, Supporting 1 in 3 Medicaid Lives in Oregon

GSI Health, a leading population health management technology company, today announced that its GSIHealthCoordinator platform has been implemented to manage care for CareOregon, a nonprofit organization supporting almost a third of Oregon’s Medicaid population. As the largest provider of Oregon Health Plan services, CareOregon supports four coordinated care organizations (CCOs) serving nearly 300,000 Medicare and Medicaid members across the state.

After a national search, CareOregon selected GSI Health as a partner to address the criticality of care coordination in its population health management strategy. The GSIHealthCoordinator platform integrates key systems and people to improve efficiency, breaking down silos and enabling collaboration among disparate teams spanning the health plan’s business lines and community-based partners. GSIHealthCoordinator helps CareOregon put analysis into action, directing how care teams work together with harmonized workflows to proactively address all issues, including lifestyle and socioeconomic factors that can affect member health.
