Dorland Health, a division of Access Intelligence, announced the winners of the fourth annual Case In Point Platinum Awards, recognizing outstanding case management professionals and organizations committed to ensuring the system is patient- and family-centric, safe and effective. The Awards Program highlighted the innovative and creative work across the healthcare system that is improving access, promoting wellness and ensuring safe transitions of care through effective care coordination across the care continuum.

A distinguished panel of judges reviewed entries across 30 categories that encompassed care coordination efforts in diverse settings, including acute care, managed care, disease management, behavioral health, social work, rehabilitation, workers’ compensation, military healthcare, wellness and prevention programs and others.

“The Case In Point Platinum Awards is a competition which strives to showcase the best case/care management programs and people, and is an important step toward raising the visibility of the profession and showing the public, lawmakers and other members of the healthcare team the important contributions case/care management makes to healthcare,” said Anne Llewellyn, editor-in chief of case management products with Dorland Health.

The Platinum Awards winners and honorable mentions were recognized at an awards brunch on May 7, 2013, at the Gaylord National Resort in National Harbor, MD.
