Diabetes America Teams Up with Chevron Phillips Chemical for an Innovative Diabetes Management Program

HOUSTON, Dec. 3 /PRNewswire/ -- Diabetes America (DA), the premier network of diabetes care and management centers across the state of Texas, announced today that they have partnered with Chevron Phillips Chemical Company LP and Aetna to provide employees with an innovative new diabetes management program. Patients or dependents over the age of 18 enrolled in the Chevron Phillips Chemical medical plan that are currently living with diabetes are eligible for the program and will be able to start utilizing the full spectrum of Diabetes America's services starting January 1, 2008.

By partnering with Diabetes America, Chevron Phillips Chemical has taken proactive steps both in promoting employee wellness and in the battle against diabetes, a chronic illness that affects seven percent of the population. It also demonstrates that the Company places a priority on employee wellness and that they are committed to keeping their workforce healthy.

Each new patient will have a minimum of five physician visits for the first year of the program and will begin a personalized treatment plan developed specifically for them by Diabetes America's expert team of physicians, registered dietitians and certified diabetes educators. In addition, patients will also have access to a whole range of services including online appointment scheduling, personalized health records and prescription refills, and retail pharmacy services with free overnight delivery.

Patients of Diabetes America will also be able to take advantage of a unique program called Diabetes America Cares, which provides free, 24/7 lifestyle and behavioral health counseling from trained health professionals.
