CDPHP Medicare Choices Expands Service Area Northward

CDPHP announced the expansion of its Medicare Choices program into three additional New York counties - Washington, Warren, and Essex - to complement its growing service area. Medicare beneficiaries in these counties will have access to the CDPHP individual Medicare Choices PPO plans, effective January 1, 2010.

Albany, N.Y. (Vocus) October 12, 2009 -- CDPHP announced the expansion of its Medicare Choices program into three additional New York counties - Washington, Warren, and Essex - to complement its growing service area. Medicare beneficiaries in these counties will have access to the CDPHP individual Medicare Choices PPO plans, effective January 1, 2010.

"CDPHP has the experience of providing preventive care, case and disease management, and other critical services to add high-quality value and service to all our plans, including Medicare Choices," said John D. Bennett, MD, president and CEO, CDPHP. "Now as we expand into more counties, we can offer these valued-added plans to an even larger service area, which include many regional hospitals and federally qualified health clinics, with no referrals needed to see specialists."

Comprehensive Support
Value-added services include such programs as CDPHP Health AllySM, a no-cost, voluntary, comprehensive program that provides Medicare Choices members and their caregivers with support, education, and access to CDPHP benefits and community-based services. A detailed health assessment pinpoints each member's need for wellness education and case management. CDPHP registered nurses and medical social workers work collaboratively with members and their physicians to determine how best to support an individ
