Avenidas Announces Programs for November and December in the Bay Area

Avenidas is offering several programs during the next two months to help the elderly and their caretakers to find the appropriate level of care.

“As we enter the holiday period from Thanksgiving to Christmas, Avenidas has resources for families caring for aging loved ones that should help ease the burden,” stated Avenidas Marketing Vice President, Kari Martell.

From 6-7:30 p.m., Wednesday, Nov. 13 will be the seminar, “From the Emergency Room to Discharge Planning.” Topics will include medical care teams, patient’s rights, the discharge process, skilled versus non-skilled care, elder abuse, and appealing discharge dates. Also in November, as part of the Family Caregiving 101 series sponsored by Avenidas Rose Kleiner Center, the Alzheimer’s Association, El Camino Hospital, Family Caregiving Alliance, and Stanford Geriatric Education Center, a no-cost workshop will focus on “Understanding Family Dynamics.” The workshop is presented by Chris Irving of Family Caregiving Alliance at 7 p.m., Thursday, Nov. 14 at Avenidas Rose Kleiner Center in Mountain View, 270 Escuela Ave. Space is limited, so RSVP by calling 650-289-5498.

“This topic is so timely, right before families gather together around the Thanksgiving table,” said Kari Martell. “Attending this workshop should really help families who are struggling to get along with each other in a difficult period.”

Paula Wolfson, LCSW and manager of Avenidas Care Partners, will give a presentation entitled “Navigating the Hospital Stay: A Caregiver’s Role,” from 11 a.m. to 12:30 p.m. Monday, Nov. 25. Call 650-289-5438 with any questions.

In December, back by popular demand, will be another Family Caregiving 101 workshop on “How to Increase Balance & Decrease Falls” by Ellen Corman, MRA and Tamar Semerjian, Ph.D., at 7 p.m. Thursday, Dec. 5 at Avenidas Rose Kleiner Center in Mountain View. RSVP to (650) 289-5498.

Avenidas will host “Bring Back the Light: Holiday Tips for Caregivers,” presented by Tiffany Mikles of Dementia Care Coaching from 6-7:30 p.m. Wednesday. Dec. 11. Participants can learn how to identify and solve problem issues that caregivers and people with dementia have around the holidays, customize previous traditions so everyone can participate and be involved, and create fun and meaningful holiday activities. Finally, join Avenidas to practice mindfulness meditation and discuss ways to help reduce stress and enhance emotional health from 1-2:30 p.m., Wednesday, Nov. 20. at the Rose Kleiner Center. RSVPs are required by calling (650) 289-5496.

Avenidas offers families a special geriatric care management package. This includes an initial meeting to discuss the situation, then a clinical social worker visits the older adult in their home and conducts a comprehensive assessment, including a dementia screening, medical history review, nutritional status, home safety issues, etc., to make sure the scope of the problem is understood. In a follow-up consultation, the written care plan is reviewed with the family, which includes the results of the assessment, recommendations, and referrals. The families can choose to implement the plan on their own, or retain Avenidas Care Partners for further assistance in arranging services and monitoring results. Avenidas also offers information and assistance by phone at 650-289-5433 or in person, providing helpful resources on topics such as housing options, services, meal delivery, etc. Their clinical social workers also offer consultations for family members to discuss their aging parents, or ongoing counseling sessions to help people deal with grief, loneliness or anxiety.
