Content focuses on: Providing an overview and update of national strategies addressing transitions of care and reduction of readmissions, specific roles physicians and their extenders can play to reduce inappropriate readmissions, transitions of care opportunities to reduce readmissions in high risk patients, and initiatives that have been successfully employed to reduce readmissions in hospitals
