Description Growth Hormone Binding Protein (GHBP) is the soluble, extra-cellular domain of the Growth Hormone (GH) receptor and is an integral component of the growth hormone-insulin-like growth factor (IGF) axis. Depending on the species, it is either a cleavage product of the GH receptor or a splice variant of the GH receptor gene. Growth Hormone receptor cleavage /GHBP shedding is effected by TNF-a converting enzyme, a metalloproteinase of the ADAM family, and possibly related enzymes (1). GHBP circulates in plasma in nanomolar concentrations, sufficient to complex approximately 50% of plasma GH. GHBP modulates GH action via a variety of mechanisms. It inhibits GH action by competing with the GH receptor for ligand and by generating unproductive heterodimers with the GH receptor at the cell surface, as opposed to the GH receptor homodimers required for signaling. The GHBP also prolongs the half-life of GH in the circulation by complexing GH and delaying its elimination (2). The biological role of the GHBP is still unclear due partly to the previous lack of reliable and convenient assays and also due to the complexity of the in vitro effects (3). The main clinical application of GHBP measurements is in the diagnosis of genetic growth hormone insensitivity (Laron syndrome), where levels are typically decreased (4).
