DST Health Solutions' Integrated Care Management Suite Improves Care Quality, Population Management of 31,000+ Member Health Plan

BIRMINGHAM, Ala., Oct. 25, 2011 /PRNewswire/ -- DST Health Solutions, LLC, today announced that San Francisco-based Chinese Community Health Plan (CCHP) has implemented its PowerMHC® core claims administration platform and Integrated Care Management (ICM) Suite to help meet new market requirements and enhance data integration. In addition, DST Health Solutions helped to implement a nurse assist line service for CCHP, which supports more than 31,000 commercial, Medicare and TPA members with health plan services.

CCHP will benefit from DST Health Solutions' ICM suite, which provides insight to properly manage populations, effectively utilizes benefits and provide education for optimal self-management. The cornerstones of the ICM Suite are DSTHS CareAnalyzer® for member identification and CareConnectTM for care management, which has the ability to be integrated with any core claims administration platform, including DST platforms. The ICM Suite leverages The Johns Hopkins ACG® System predictive model, which identifies members with the greatest opportunity to improve health outcomes and identifies gaps in care. In addition, the DST Health Solutions approach offers rich analytics and decision support to help users meet the Healthcare Effectiveness Data and Information Set and Five-Star quality rating requirements.
